Job Summary

This clinical social worker position works with the Positive Health Access to Services and Treatment (PHAST) and RAPID teams. The purpose of the PHAST program is to provide enhanced support with rapid linkage, HIV treatment, and sustained engagement in primary medical care through integrated nursing and medical case management, outreach services, social services, and coordination of care with appropriate community agencies. For newly diagnosed HIV positive individuals and patients who have been off antiretroviral therapy, identified either at the Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG) campus or community testing sites, the PHAST/RAPID team provides enhanced supportive interventions, such as HIV test disclosure, post-test counseling and education, partner services, engagement interventions, and facilitates immediate access to medical care and HIV treatments, often on the same day of diagnosis or identification. Incumbent will work with patients receiving services from the PHAST/RAPID program, will see patients in a variety of settings, will assess their psycho-social needs, and design and execute comprehensive psychosocial care plans to support engagement in HIV care.

Incumbent will collaborate closely with other members of the interprofessional PHAST/RAPID team and the inpatient Infectious Disease Consult Service and serve as a liaison between ZSFG and the outpatient HIV medical clinic, the Emergency Department social services staff, and other key community primary care clinics and organizations. Under the direction of the Social Work Supervisor, the incumbent is responsible for providing comprehensive psychosocial care to persons living with HIV. Incumbent will additionally work with HIV positive individuals on implementing structured interventions to support ART adherence through both RAPID and RAPID Restart pathways. The RAPID Restart program is a new initiative aimed at providing enhanced support for individuals who experience challenges with ART adherence and will provide opportunities to participate in developing clinical interventions, programmatic evaluation, and develop subject matter expertise in a new area of HIV care. Incumbent will also have responsibilities coordinating key collaborations within the DPH, coordinate training and technical assistance with other clinical sites, and participate HIV treatment and prevention efforts, committees, and collaborations as needed.

The Division of HIV, Infectious Diseases and Global Medicine at UCSF is an internationally recognized global leader in clinical care, research and education. With its home-base located at ZSFG, the Division has been ranked as the top facility in the country for AIDS care for over ten years. The Division's work is featured frequently in the media, and it is often visited by international, national, state and local dignitaries and delegations. The Division consists of approximately 36 faculty members and 140 academic and staff employees. It is the largest Division of the Department of Medicine at ZSFG, with a total expenditure budget of approximately $45 million dollars.

Required Qualifications

Clinical Social Worker 1:
* Master's degree from an accredited school of social work; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position.
* Fluency in Spanish - ability to communicate effectively with monolingual patients.
* Ability to work in a high stress, high volume area and work well under pressure and with deadlines.
* Ability to handle demanding workload involving multiple tasks effectively and efficiently.
* Ability to function independently as a collaborative, supportive team member.
* Must demonstrate cultural humility and a strengths-based, recovery philosophy.
* Demonstrated excellent oral/telephone communication skills and written documentation.
* Must be computer-literate, working knowledge of HER systems, and Microsoft Suite.
* Excellent organizational and strong interpersonal skills
* Demonstrated excellent attendance and reliability

Clinical Social Worker 2:
* In addition to the above, a Master's degree from an accredited school of social work, and possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position

License/Certification

Active license with the California Board of Behavioral Sciences as an Associate Clinical Social Worker (ASW) or equivalent.

Clinical Social Worker 2: In addition to the above:

  • Possession of the requirements necessary for application for licensure as a Licensed Clinical Social Worker as determined by the California Board of Behavioral Science Examiners; or equivalent
